A restaurant has been ordered shut for the third time for a roach infestation.

4 CRITICAL VIOLATIONS

1 NON CRITICAL VIOLATION 

“Critical. Observed roach activity as evidenced by 25+ live roaches found in the kitchen in wall cracks, shelves and crawling on the walls and floors. Repeat Violation 3rd time.” 

“Observed take-home food container refilled with potentially hazardous food. storing ketchup in garlic container.” 

“Critical. Potentially hazardous food not held at 135 degrees Fahrenheit or above. pastry hot box, beef pastry Corrected On Site.” 

"Critical. Observed food stored on floor.” 

“Critical. Ready-to-eat, potentially hazardous food prepared on site and held more than 24 hours with not properly date marked.”
