PEMBROKE PARK, Fla. - NASA has admitted to making a dangerous mistake, following a near-drowning incident in space last summer, according to ABC News reports.

An Italian astronaut almost drowned during the second spacewalk of his career in July 2013 after his spacesuit leaked. The leak caused his helmet to fill with several liters of water.

NASA says the same spacesuit had leaked just a week before, but details of the initial incident did not make its way up the chain of command.
