According to a Florida Highway report, Jacobs tried to leave the scene of the crash and had her two children -- a 14-year-old daughter and a 10-year-old son -- in the car at the time.

A state trooper wrote in the report that Jacobs "had a slight sway and appeared unable to maintain her balance." The report said that Jacobs smelled of alcohol, had bloodshot eyes and a slurred speech.

According to the report, Jacobs told the trooper that she had three glasses of wine at Outback Steakhouse before picking up her children from Cooper City.

When the trooper asked why she tried to leave the scene, Jacobs said "she felt nothing had occurred and left," the report said.

After Jacobs performed a field sobriety test and was placed under arrest, she "began to act very irate and used abusive language multiple times," the trooper wrote.

"On multiple occasions, she advised me (that) she would make sure that I lost my job over this incident," the trooper wrote.

The report said Jacobs refused to consent to a breath test and refused to provide a urine sample.

Jacobs faces multiple charges, including two counts of DUI causing property damage, two counts of child neglect and one count of leaving the scene of a traffic crash.

A spokesman for the Miami-Dade County state attorney's office declined to comment on the arrest.
