PEMBROKE PARK, Fla. – State records show inspectors with the Florida Department of Business and Professional Regulation ordered five restaurants in South Florida shut last week.
Among them was a Wendy's on Oakland Park Boulevard and a huge Japanese buffet in North Bay Village.
Jack's Bar-B-Q Smokehouse was also ordered shut last week because of a rodent issue. Jack's had been ordered shut last November as well for the same issue.
All the places listed below were allowed to re-open following ordered cleanups and re-inspections.

North Bay Village
"Rodent activity present as evidenced by rodent droppings found. Observed fresh and moist rodent droppings throughout the establishment. Observed approximately 30 fresh and moist droppings on top of dish machine locate in the kitchen, approximately 75 fresh and moist droppings on top of dry storage shelves, on boxes located inside dry storage located in the kitchen and approximately 70 more fresh and and moist rodent droppings along the floor boards, shelves where clean dishes are stored and floor on the kitchen."
"Food stored on floor. Observed fish, chicken, onions, shrimp and cooked chicken on kitchen floor."
"Accumulation of black/green mold-like substance in the interior of the ice machine."
"Wall soiled with accumulated black debris in dishwashing area."
"Interior of oven has heavy accumulation of black substance/grease/food debris."
"Interior of microwave soiled with encrusted food debris."
"Certified food manager or person in charge lacks knowledge of food borne illnesses and symptoms of illness that would prevent an employee from working with food, clean equipment and utensils, and single-service items."

"Roach activity present as evidenced by live roaches found as evidence of approximately 20 in the ware washing area, food storage area under cracked tile in front of reach in cooler and kitchen area."
"Roach excrement and/or droppings present. Too many to count in ware washing area on shelves that has pots."
"Operator unable to provide documentation for source of fish. See stop sale. Have swai on site."
"Stop Sale issued on potentially hazardous (time/temperature control for safety) food due to temperature abuse. Rice 55 degrees Fahrenheit, meat 54 degrees Fahrenheit, sour cream 53 degrees Fahrenheit, queso blanco shredded cheese 56 degrees Fahrenheit, 2 dozen eggs."

"Roach activity present as evidenced by live roaches found. Observed approximately 11 live roaches inside kitchen at the mop sink and on prep table where fresh baked bread is held."
"Stop sale issued on potentially hazardous (time/temperature control for safety) food due to temperature abuse. Observed lentil soup made from previous day at 62 degrees Fahrenheit."

"Dead roaches on premises. 1 dead on floor between hand wash sink and frosty machine at front counter. 1 dead on floor at cook line under green rolling shelf. 3 dead on floor at drive thru window. 1 dead on floor behind soda machine at drive thru. 3 dead on floor under three compartment sink. 1 dead on floor between oven and wall in back prep area. 1 dead on wall behind shelf holding chemicals next bread racks. 1 dead under shelf hold case of soda next to large C02 tank. 1 dead in light shield next to walk in cooler. 1 dead in light shield between fryer and drive thru. 1 dead in light shield over tea and coffee station next to monitor. 1 dead on floor at hallway under bug zapper."
