Restaurant ordered shut for 3rd time; another had live roach on deli case

Below is a list of places inspected last week by the Florida Department of Business and Professional Regulation.

Pan American House was not ordered shut but did have roach and cleanliness issues.

All the places ordered shut were allowed to re-open following an ordered clean-up and re-inspection.

No places were ordered in the Florida Keys.

“Rodent activity present as evidenced by rodent droppings found. Observed 28 rodent droppings in the following areas: 23 rodent droppings in middle cabinet of front service line. 1 rodent dropping in middle drawer above cabinet at front service station. 4 rodent droppings on top of dishwashing machine.”

“Dead roaches on premises. Observed one dead roach under triple sink and 1 dead roach in cabinet in front coffee making station area.”

“Interior of oven/microwave has accumulation of black substance/grease/food debris. Microwave in prep area soiled. Also, interior of bottom oven soiled.”

“Raw animal food stored over/not properly separated from ready-to-eat food. Raw chicken stored above cooked noodles in walk-in-cooler. Also, raw shelled eggs stored over pineapple in walk-in-cooler. Operator removed items to properly store.”

“Single-service articles stored in toilet room/locker room/garbage room/mechanical room. Observed pizza boxes stored inside bathroom.”

“No running water at three-compartment sink. Observed no running water at triple sink, in kitchen.”

“Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. Observed no time marking on pizza slices. Per operator, pizza made at 12:30pm. Operator put time marking on pizza slices.”

“Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed chicken wings prepared onsite stored inside reach in cooler, no date marking.”

“Employee applied hand antiseptic in place of washing hands as required. Observed operator pour isopropyl alcohol onto hands while wearing gloves, to sanitize hands. Operator then proceeded to grab pizza paddle.”

“Hand wash sink not accessible for employee use at all times. Observed ziploc boxes stored inside kitchen hand wash sink.”

“Hand wash sink used for purposes other than hand washing. Hand wash sink in kitchen used for ware washing.”

“No proof of required state approved employee training provided for any employees.”

“Roach activity present as evidenced by live roaches found. Observed approximately 56 live roaches in the following areas: 20 under triple sink in back of restaurant 4 at slicer 6 under oven in back room 6 under water heater 10 under mop sink 10 between wall and reach in freezer in back kitchen area.”

“Dead roaches on premises. Observed one dead roach in hall leading to employee restroom.”

“Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Interior of ice machine in prep area soiled. Repeat Violation.”

“Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. milk (45F - Cold Holding) in glass door reach-in cooler at front service line. Employee stated the milk was out to make a few coffees and then placed back in cooler. Employee will leave milk in cooler to chill. Also, cooked pork (45F - Cold Holding) in reach-in cooler in prep area. Operator stated the pork may have been out after making empanadas. Rest of temps in same reach-in cooler 41F and below. Operator took lid off container to chill pork.”

“Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. cooked fish (95F - Hot Holding); cooked black beans (97F - Hot Holding) in front display food warmer. Operator turned up warmer in area of fish and beans.”

“Food-contact surface soiled with food debris, mold-like substance or slime. Slicer and blade of slicer soiled. Also, cutting board at reach-in cooler at front service line and cutting board in prep area soiled. Repeat Violation.”

“Raw animal food stored over/not properly separated from ready-to-eat food. Raw shelled eggs stored over ready to eat black beans in walk-in-cooler. Operator removed black beans to properly store.”

“Employee washed hands with no soap. Employee washed hands without using soap. No soap available at start of inspection for employees to use. Operator placed soap at hand wash sink and employees able to use soap when washing hands.”

“Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee touched dirty dishes and then touched clean dishes and utensils without washing hands in between. Also, employee touching phone and then touched utensils to serve food without washing hands.”

“No currently certified food service manager on duty with four or more employees engaged in food preparation/handling.”

“No proof of required state approved employee training provided for any employees.”

“Observed 20 dead roaches in the following areas: 1 dead roach under coffee equipment at front service area 2 dead roaches under reach in deep freezer across from ice machine 3 dead roaches under triple sink in prep area 2 dead roaches under shelves with beer 4 dead roaches under reach-in cooler/ freezers behind prep area 6 dead under dry food storage area 1 dead in employee restroom 1 dead in hall area leading to employee restroom.”

“Observed 11 live roaches in the following areas: 1 live under ice machine in prep area 1 under clean equipment storage shelf in prep area 2 live under soda storage shelves in back kitchen 1: live under small pizza oven in back room 1 live at water heater 1 live on wall at triple sink 3 live at reach in freezer behind prep area 1 live on floor in back kitchen area.”

Observed fried rice (52F - Cold Holding) and cooked chicken (58F - Cold Holding) stored on top of the kitchen counter, as per operator taken out of the walk in cooler 2 hours before inspection. Operator placed back inside walk in cooler. Repeat Violation.”

“Food-contact surface soiled with food debris, mold-like substance or slime. Observed can opener blade soiled.”

“Equipment and utensils not washed, rinsed and sanitized in the correct order in three-compartment sink. Do not use dishes/equipment not properly sanitized. Observed employee washing a pan and placing pan on the drying rack without sanitization. Educated employee how to properly wash, rinse and sanitize correctly.”

“Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed cook line with accumulation of grease. Observed gaskets in reach in cooler soiled. Repeat Violation.”

“Wall soiled with accumulated grease, food debris, and/or dust. Observed wall soiled by the 3 compartment sink. Repeat Violation.”

“Dead roaches on premises. Observed 2 dead roaches in bar area and 3 dead roaches in cabinets below espresso machine at service line. Operator cleaned up and discarded dead roaches.”

“Stop Sale issued due to food not being in a wholesome, sound condition. One live roach on lettuce in deli case.”

“Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed approximately 12 flying insects in the following areas: 6 flying insects in bar area, 4 flying insects in hall where liquor stored leading to bar area, and 2 flying insects in deli area. Repeat Violation.”

“Employee used tobacco, ate, or drank then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Two Employee ate and then touched clean utensils without washing hands.”

“Floor soiled/has accumulation of debris. Floor heavily soiled under equipment and shelving throughout kitchen, cook line, prep areas and bar area.”

“Food-contact surface soiled with food debris, mold-like substance or slime. Cutting board on cook line and cutting board in deli area on reach-in cooler soiled/ stained. Also, soda gun and holster soiled in bar area. Also, slicer guard and slicer blade soiled with an accumulation of dried food debris. Repeat Violation.”

“Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. cream cheese (46F - Cold Holding) in top of flip top reach in cooler across from dessert case. Employee stated the lid was left open for a little while. Operator removed cream cheese and placed in nearby cooler to chill. Also, Liquid eggs (46F - Cold Holding) in bottom of reach-in cooler across from dessert case. Employee stated she had eggs out for about 10 minutes. Operator discussed with employee to keep eggs in cooler after each use. Also, slice American cheese (50F - Cold Holding) in reach in cooler across from grill. Observed cheese stored above chill line of cooler. Employee removed excessive cheese and placed in bottom of reach in cooler. Also, house made tuna salad (48F - Cold Holding); house made chicken salad (47F - Cold Holding) in deli case next to hot bar. Operator stated both items were made three days ago and unsure when reach-in cooler case stopped being able to maintain temperature of 41F and below. Repeat Violation.”

“Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. House made chicken salad and house made tuna salad prepared 3 days ago per operator not labeled.”

“No currently certified food service manager on duty with four or more employees engaged in food preparation/handling.”

“Stop Sale issued due to food not being in a wholesome, sound condition. Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed two packages of salmon still in the vacuum at an internal temperature of 39°F.”

“Dish machine not sanitizing properly. Discontinue use of dish machine for sanitizing and set up manual sanitization until dish machine is repaired and sanitizing properly. Observed the dish machine sanitizer at 0.00. The cook set the triple with quaternary sanitizer at 200 ppms to sanitize.”

“Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. Observed chicken over vegetables in the upright freezer. The cook put the chicken on the bottom shelf.”

“Employee used tobacco, ate, or drank then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed at the front counter.”

“Hand wash sink not accessible for employee use at all times. Observed a rack of utensils on top of the hand washing sink next to the dish machine. The cook moved it.”

“No proof of required state approved employee training provided for any employees.”

Observed raw pork (60F - Cold Holding); raw chicken (50F - Cold Holding); raw chicken (50F - Cold Holding) in reach in cooler located right across cook line kitchen. As per staff placed there since previous day. Observed ham croquets (51F - Cooling); turkey ham (47F - Cooling); ham empanadas (47F - Cooling) located in walk in cooler. As per operator items were prepared since previous day.”
