One place was shut down for the third time over a rodent problem and this time, inspectors say they even found a dead rat.

Rodent issues showed up in at least three South Florida restaurants last week, including Pack Supermarket in Miami, Georgie’s Alibi Monkey Bar in Wilton Manors and Azumare in Aventura, and that’s just the start.

At Pack Supermarket, inspectors found droppings sitting next to utensils and food prep areas, and also noted tomato sauce being held at unsafe temperatures.

At Takato in Fort Lauderdale, dozens of small flying insects were landing on clean plates and sushi mats.

And at Restaurant Brands International in Miami, there was a mold-like buildup inside an ice machine used for drinks.

Over at Origen in Cooper City, an employee handled raw chicken and then went straight to ready-to-eat food without washing their hands.

Inspectors also found standing water on floors at Pack Supermarket, grease caked behind equipment at multiple locations, and ceiling tiles with damage and holes hanging over prep areas.

And that repeat shutdown?

That was Pack Supermarket in Miami, which has now been closed three times for similar problems.

All the places, except Origen in Cooper City, have since been allowed to reopen after a required cleanup and re-inspection.
