A Dirty Dining violations trifecta leads this week’s list!

A dead rodent, roaches and flies were all found inside one South Florida restaurant.

At Achsah’s Jamaican Restaurant/Bakery in Fort Lauderdale, inspectors discovered a dead rodent on the floor, live roaches crawling on walls and inside clean dishes, and swarms of small flying insects landing on food.

Meanwhile, in Cooper City, Origen remains shut down for more than a week as roaches keep showing up during follow-up inspections. And in Hialeah, Mesa’s BBQ racked up an eye-popping 46 violations, including flies landing on food and hundreds of pounds of improperly handled pork.

Below is a list of places and some of their violations.

All the places, with the exception of Origen, have been allowed to re-open following an ordered cleanup and re-inspection.

“Dead rodent present. Observed a dead rodent in the bathroom on the floor. Operator disposed of the dead rodent.”

“Roach activity present as evidenced by live roaches found. Observed 2 live roaches near the handwashing sink crawling on the walls and inside of clean dishes. Observed 2 more live roaches crawling on top of the reach in freezer.”

“Dead roaches on premises. Observed 2 dead roaches at the dry storage counter. Observed 1 dead roach in the handle of the garbage can lid. Observed 3 dead roaches under the front counter steam table on the floor. Observed 2 dead roaches in the electrical closet area.”

“Live, small flying insects found. Observed approximately 6 small flying insects by the handwashing sink landing on dirty and clean dishes. Additionally observed approximately 8 small flying insects landing on raw plantains and potatoes in the dry storage area. Observed approximately 5 small flying insects in the electrical closet area landing on cleaning supplies and walls.”

“Pesticide/insecticide labeled for household use only present in establishment. Observed operator use raid bug spray after inspector pointed out live roaches found near the handwashing sink.”

“Handwash sink used for purposes other than handwashing. Observed operator washing dishes in the handwashing sink. Advised operator to wash dishes in the triple sink and to only wash their hands in the handwashing sink.”

“Manager or person in charge lacking proof of food manager certification.”

“No proof of required state approved employee training provided for any employees.”

“Operating with an expired Division of Hotels and Restaurants license. Observed license expired Dec 1, 2025.”

Also observed 6 flying insects flying inside orange juice machine at front counter. Also observed 2 flying insects landing on 2 pieces of bread on top of preparation table at front area.”

“Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed ice machine interior soiled with mold like substance next to walk in cooler at back area.”

“Stop Sale issued due to food not being in a wholesome, sound condition. Observed 2 flying insects landing on 2 pieces of bread on top of preparation table at front area. Also observed approximately 200 pounds of precooked pork belly inside walk in cooler and walk in freezer with no written procedures approved by the division. As per operator prepared 2 days ago at a temperature of 130F to 135F. Stop sale issued.”

“Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed Cooked pork (53F - Cooling) on shelf inside walk in cooler at 9:38am. As per operator pork was cooked and kept since 11pm the night before inside oven and was moved inside walk in cooler at 9 am this morning.”

“Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed Cooked Yuca (126F - Hot Holding) Assorted croquettes (96F - Hot Holding); Cooked whole pork (105F - Hot Holding); Cooked Pork (116F - Hot Holding); Baked Chicken (116F - Hot Holding); Baked pork belly (126F - Hot Holding). As per operator for less than 4 hours.”

“Food-contact surface soiled with food debris, mold-like substance or slime. Observed slicer machine soiled on top of table in front of ice machine.”

“Wall soiled with accumulated grease, food debris, and/or dust. Observed walls soiled.”

“Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed reach in cooler gaskets soiled at kitchen area. Repeat Violation.”

“Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed throughout kitchen.”

“Employee touched bare body part and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee touching face repeatedly and continuing food prep. Inspector stepped in and coached employee on proper handwash techniques, HEAL side.”

“Raw animal foods not properly separated from one another based upon minimum required cooking temperature when stored in a freezer - not all products commercially packaged. Observed raw tuna filets on top of ice cream in reach in freezer.”
