Filthy Pho restaurant ordered shut, plus a Carrot Express makes Dirty Dining list

BROWARD COUNTY, Fla. ā€“ Below is a list of places that were inspected last week and ordered shut by the Florida Department of Business and Professional Regulation.

All the places mentioned were allowed to re-open following an ordered cleanup and re-inspection.

ā€œTime/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. -Observed quinoa hot held at 104 degrees Fahrenheit in kitchen. Per operator, item placed in unit 3.5 hours prior to the inspection. Operator reheat item to 173Ā°F.ā€

ā€œFood-contact surface soiled with food debris, mold-like substance or slime. -Observed accumulation of old food residue buildup on can opener blade.ā€

ā€œFood stored on floor. -Observed cases with various produces stored on floor at front line area. -Observed containers with cooking oil stored on kitchen floor.ā€

ā€œHand wash sink used for purposes other than hand washing. -Observed a food container stored inside hand wash sink at front line.ā€

ā€œNo currently certified food service manager on duty with four or more employees engaged in food preparation/handling.ā€

ā€œRaw animal food stored over/not properly separated from ready-to-eat food. -Observed raw chicken wings stored over salad in walk-in cooler.ā€

ā€œTime/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. -Observed cooked pork cold held at 44 degrees Fahrenheit in flip top cooler. Per operator, item placed in unit 30 minutes prior to the inspection. Operator placed ice pack on item for quick.ā€

ā€œFood being cooled by non-approved method as evidenced by inadequate rate of cooling during time of inspection. -Observed containers with soups covered while cooking in walk-in cooler. Evidence by condensation on lids. After 6 hours, soups temps at 47-48Ā°F.ā€

ā€œBuildup of food debris/soil residue on equipment door handles. -Observed accumulation of soiled residue buildup on reach in coolers doors.ā€

ā€œFloor not cleaned when the least amount of food is exposed. -Observed accumulation of old food residue buildup underneath cooking equipment and floor throughout kitchen.ā€

ā€œNonfood-contact surface soiled with grease, food debris, dirt, slime or dust. -Observed accumulation of old food residue buildup and grease buildup on shelves underneath food preparation tables throughout kitchen.ā€

Approximately 5 live roaches inside bin containing packages of dried pasta on dry storage shelf in kitchen. Bin removed from building.ā€

ā€œDead roaches on premises. 5 dead roaches under front counter in dining room.ā€

ā€œTime/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. At the cook line prep table, hash brown potatoes (70F - Cold Holding). Item being held at room temperature to be used to order. Item returned to refrigerator to chill and cold hold.ā€

Dish area-observed approximately 10 live flies flying over trash can and dirty dishes. -dish area- observed approximately 20 live flying insects flying over and landing on clean utensils. -dry storage shelf- observed 5 live flying insects flying around single service napkins.ā€

ā€œStop Sale issued due to adulteration of food product. Bar- observed approximately 25 live flying insects flying around and landing on cutting board, inside ice bin and landing on syrup containers squeeze bottles.ā€

ā€œPesticide/insecticide labeled for household use only present in establishment. Front counter at cold holding unit- observed spray bottle of Hot Shot insecticide on shelf at register.ā€
