Roaches crawling around 5 South Florida restaurants lead to ordered closures

Below is a list of places that were ordered shut by inspectors with the Florida Department of Business and Professional Regulation.

All the places mentioned were allowed to re-open following an ordered cleanup and re-inspection.

“Roach activity present as evidenced by live roaches found. Observed 1 live roach on wall in left side stall of women’s rest room. Operator removed cleaned and sanitized. Observed approximately 2 live roaches inside of metal trim around cut out in wall for air flow behind cook line flip top cooler on cook line.”

“Dead roaches on premises. Observed 3 dead roaches on ground under expo line table on opposite side of wall to cook line flip top. Observed 1 dead roach on ground on cook line in kitchen.”

The restaurant’s corporate office released the following statement to Local 10 News Tuesday:

“We are aware of the pest sighting observed last week in one of our locations. We immediately worked with our pest control vendor to address this issue, along with reviewing our internal controls. For years we have serviced our restaurants successfully twice a month. This issue has been resolved and we have taken additional steps to ensure our locations have added treatments scheduled beyond the normal scope of service. We are confident this is an isolated incident. We pride ourselves in having sanitation and food safety as our number one priority and look forward to continuing to earn our communities’ confidence and business.”

Observed 4 live roaches crawling on the kitchen floor behind reach in cooler, 2 live roaches crawling on the floor in the kitchen area by the walk-in cooler door, 4 live roaches crawling on the floor of the dry food storage area, 1 live roach crawling on the floor by the kitchen exit door.”

“Dead roaches on premises. Observed 30 dead roaches on the floor underneath reach in cooler, 3 compartment sink, and under dry food storage shelving . Also 5 dead roaches by the hand sink at kitchen entrance from dining area.”

“Tracking powder pesticide used inside establishment. Observed tracking powder in use all throughout establishment.”

“Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed oven interior soiled with old food debris. Repeat Violation.”

“Floor area(s) covered with standing water. Observed standing water on the floor around the reach in cooler. Repeat Violation.”

“Floors not maintained smooth and durable. Floor not maintained smooth or durable. Repeat Violation.”

“Roach excrement and/or droppings present. Observed 1 roach egg on top of a can of tomato sauce also approximately 20+excrements on the shelf lining the wall behind the prep table in the kitchen nearby the hand washing sink.”

“Time/temperature control for safety food thawed in an improper manner. Observed bags of raw chicken, raw beef and raw pork thawing in water inside of triple sink. As per chef, placed inside the sink 1 hour before the inspection.”

“Food-contact surface soiled with food debris, mold-like substance or slime. Observed deli slicer and in use cutting boards soiled. All food containers inside of reach in coolers and prep shelves soiled.”

“Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling tiles soiled in kitchen area.”

“Wall soiled with accumulated grease, food debris, and/or dust. Observed wall soiled with grease throughout entire kitchen. Repeat Violation.”

“Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed microwave and microwave door at front counter soiled.”

“Hand wash sink not accessible for employee use at all times. Observed strainer with raw shrimp stored inside of hand wash sink next to triple sink inside of kitchen. Observed hand wash sink by front counter blocked by shelf.”

“Hand wash sink used for purposes other than hand washing. Observed hand wash sink being used to rinse soiled wet wiping cloth at front counter.”

“Employee has not received adequate training related to their assigned duties as evidenced by lack of general knowledge in performing their assigned duties. Observed employee using cellphone then proceeded to handle and serve food to customer without washing hands. Advised employee to wash hands before serving food to customers. Observed employee washing hands inside of triple sink. Observed employee rubbing hands for less than 10 seconds while washing hands.”

“Bowl or other container with no handle used to dispense food. Observed bowl with no handle being used to scoop rice in preparation area.”

“Ceiling/ceiling tile shows damage or is in disrepair. Observed ceiling tiles broken and with water stains throughout the establishment.”

“Perimeter walls and roofs do not effectively protect establishment against environmental cross contamination or the entrance of pests. Observed back door which leads to kitchen area not attached, door is resting inside reach in cooler inside the establishment.”

“Reach-in cooler interior/shelves have accumulation of soil residues. Observed all reach in coolers with soiled shelves. Repeat Violation.”

Observed about 6 dead roaches floating in water in a tray under the three compartment sink at cook line, observed one dead roach on the floor by the garbage can at cook line, observed 1 roach dead under the pizza oven in the kitchen, observed 3 dead roaches on the floor by the kitchen entrance, observed 1 dead roach on the floor at front line across from the three compartment sink, observed 1 dead roach on floor by kitchen entrance.”

“Pesticide/insecticide labeled for household use only present in establishment. Observed Orthene Pesticide spread throughout the kitchen area and front line area under the equipment and around the kitchen cove molding.”

“Accumulation of black/green mold-like substance in the interior of the ice machine/bin, located at preparation area.”

“Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed oven interior with old grease buildup at cook line.”

“Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In the walk in cooler observed cooked rice (50 F - Cold Holding); pulled pork (47F - Cold Holding); as per cook food was outside out of temperature control for less than 4 hours then placed back in the walk in cooler to rapidly cool.”

“Food manager certification expired.”

“Hand wash sink used for purposes other than hand washing. Observed a wiping cloth stored in the hand sink at front line.”
